Abstract:

The limits of the increase of the public intervention in the social services and the rapid social –demographic changes in Western Europe have generated doubts about the existing models of social assistance and have fostered some significant reform. Many European countries have adopted innovative policies to find a better balance between the need to provide more complete care services to the population and the desire to contain the public expenditure. In some countries (such as Germany and Austria) public insurance schemes have been introduced to guarantee care to disabled people on universal basis. In other countries, such as France, the introduction of national programmes of assistance to disabled people has been accompanied by the development of measures finalised to fundraise caregivers or the purchase of services on the private market. In the Scandinavian countries, such as Sweden, the targeting criteria have been limited restricting the access to public care services. In the Mediterranean countries, such as Italy, some measure finalised to recognise and give the financial support to the social assistance of families have been introduced at local level, while a national policy is still lacking. All these measures, through different solutions, indicates a radical change in social policy. These solutions agree on the following points:

.. the tentative to combine the transfer of money to families with the provision of inkind services with the aim to support familiar and informal care (cash and care programs);

.. the start up of a new social care market, which is based on the distinction among financing (State responsibility), purchasing (totally or partially entrusted to citizen) and management of services (mostly decentralised to private agencies, profit or non profit), and on competitive procedures and market mechanisms in the relationship among financier, purchasers and suppliers;

.. the empowerment of the users’ freedom of choice through more purchasing power, and a strong legitimation of the “client” position;

.. the introduction of funding measures finalised to recognize and to foster the caregiving provided through family networks. This paper presents the main results of a research carried out in five European countries (France, Germany, Italy, United Kingdom, the Netherlands), which was aimed to identify the most significant reforms that have been introduced in Western Europe in the elderly care field in the last ten years. It analyses, on the basis of data available and interviews, the impact of such reforms on the welfare mix structure of social care, on the position of informal caregivers and care professionals, on the caregiving attitudes of familiars and the satisfaction of disabled people.
