Addressing the Challenge of Informed Decision-Making in Prostate Cancer Community Outreach to African American Men

Abstract:

Prostate cancer (CaP) disproportionately affects African American men in terms of knowledge, access to care, prevalence, and mortality. The primary available means of prevention is screening, despite the absence of evidence demonstrating screening efficacy in reducing mortality. We adopted a community-based participatory approach to design and pilot-test an outreach strategy to promote informed decision-making about CaP screening among African American men. In-depth interviews with local content experts informed the content and design of the outreach strategy, which was pre-tested with two groups of African-American men for feedback. The revised strategy was pilot-tested with 63 men who completed pre- and post-tests that measured knowledge, subjective norms, behavioral beliefs, decision self-efficacy and screening intention. We observed statistically significant improvements in knowledge, decreased perceived barriers and increased screening decision self-efficacy. This project devised an outreach strategy that balanced the imperatives of informed decision-making goals with the pragmatics of community settings.
